Each submission should include an (initial) nomination completed by the person making the nomination and including:
- the name of the person being nominated;
- the name of the person making the nomination; and
- a (brief) outline of the reason they are making the nomination.
[While you may nominate someone yourself, nominations are generally initiated by someone other than the local champion (usually a colleague, manager or offender with whom they work)].
Please complete a supporting documentation form for ALL initial nominations forwarded to you. The form has three main elements:

Your comments are intended to help build on the initial nomination – however, depending on how clear and comprehensive the initial nomination is, you may not need to add much (and please don’t repeat what’s already been said).
Please consult widely (ideally with both staff and offenders, including the nominator themselves) to ensure a full and balanced picture of the nominee/s and the work for which they are being nominated, and please read the guidance on the forms carefully.
Please avoid jargon and acronyms wherever possible, and if you have to use them, please explain them carefully as not all our Awarding Panel will be expert in your particular area.
Please ensure all nominations are signed off before forwarding them to us – the form itself details who should do this.
The forms can be completed electronically, or printed out and filled in by hand. If they are being completed electronically, please download to a computer first to ensure any changes save properly.
