Each submission should include an (initial) nomination form completed by the person making the nomination and including: the name of the person being nominated; the name of the person making the nomination; and a (brief) outline of the reason they are making the nomination.
The form can either be saved to a computer and completed electronically, or printed out and filled in by hand, via the awards & how to nominate page on the main site. [Note: although we encourage people to use the official nomination form where possible, we are happy to accept nominations from offenders in other formats too.]
Please complete a supporting documentation form for ALL initial nominations forwarded to you. The form has three main elements:

Your comments are intended to help build on, and complement, the initial nomination – however, depending on how clear and comprehensive the initial nomination is, you may not need to add much to it (and please do not repeat what has already been said). Please consult as widely as possible (ideally with both staff and offenders, including the nominator themselves) to ensure a full and balanced picture of the nominee/s and the work for which they are being nominated, and please read the guidance on the forms carefully.
